HOW TO APPLY FOR THE UPCAT Online

HERE7S HOW TO APPLY FOR THE UPCAT ONLINE AND MANUAL APPLICATION. The deadline for applicants from public schools in Metro Manila is set five days after on July 27, 2015.

Meanwhile, applicants from non-Metro Manila schools must submit on or before July 29, 2015. UPCAT application forms were made available online on Friday, July 17.



Carefully read the information and instructions provided in this Bulletin.
  1. There are two forms that have to be submitted. You have the option of filling out the application Form 1 (UPCAT Application for Freshman Admission) online or manually. Form 2 (Secondary School Record) has to be filled out by your high school principal or school head, then you must submit it with Form 1.
  2. For Online application of Form 1:
    1. Visit the UPCAT website (www.upcat.up.edu.ph) for instructions on how to fill out the Online UPCAT Application Form1 and subsequent steps in filing your application.
    2. Fill out UPCAT Form 1 online at http://upcatonline.up.edu.ph/. You will be asked for an email address which you will later use for logging on to your application account. This email address will also be used to send you notifications regarding the status/progress of your application, as well as the link to the UPCAT Online Reviewer.
    3. Upon completing the UPCAT Form 1, you will be e-mailed a printable copy (.pdf format) of the accomplished form as well as your UPCAT Form 2. Print these forms and sign in the appropriate boxes provided for your signature.
    NOTE. If you do not have a computer and/or have limited internet access you may go to the Office of Student Affairs, the Learning Resource Center, or the Main Library, of a UP Campus near you and ask for assistance. You may also go to the Information Technology Development Center in UP Diliman for internet assistance.
    For Manual application of Form 1:
    1. You may get application forms through:
      • your authorized school representative, OR
      • directly from the Office of Admissions in UP Diliman, OR
      • from the Office of the University Registrar in a UP campus outside UP Diliman.
    2. Fill out Form 1 completely and sign in the appropriate boxes provided for your signature.
  3. Give UPCAT Form 2 (Secondary School Record) to your high school principal or school head. Only a duly authorized school official may fill out this form.
  4. Pay the UPCAT Application fee (see Sec. III. Modes of Payment) or if exempted, prepare the originals and photocopies of the necessary documentary proof for Exemption (see Sec. II.3.b. Exempted).
    1. If paying thru LBP or PNB, accomplish 3 deposit slips and have them validated, then attach 1 validated slip to your application.
    2. If paying with MC/CC, attach the MC or CC (payable to UPCAT) to your application.
    3. If you are qualified for exemption, attach supporting documents (i.e. photocopies of the Income Tax Returns of earning members of your family, BIR Certification of Exemption, or certified Top Ten List) to your application.
  5. Submit the completed Forms 1 & 2 and all other required documents (see Sec. II for required documents) to the UP Office of Admissions, University of the Philippines, 1101 Diliman, Q.C. either in person, OR through the authorized school (not a bank)representative, OR by mail. You can also submit your completed forms and all other required documents to selected regional UPCAT application filing centers in: UP Baguio, UP Cebu, UP Iloilo, UP Los Banos, UP Mindanao, UP Pampanga and UP Tacloban.
    Include a self-addressed stamped envelope or return courier service if the Test Permit is to be mailed back to you. Be sure to use your school address.
  6. You will be notified by email regarding the status of your application and the details of your Test Permit. You may also log on to your UPCAT Online Account (upcatonline.up.edu.ph) to monitor the status of your application and possible deficiencies you need to attend to.
  7. Claim your Test Permit. Remember the date, time and place of your UPCAT session.
    1. For applications filed through an authorized school representative, pick up your Test Permit from this representative (usually a week or so after the date of submission to the Office of Admissions).
    2. For personally filed applications, pick up your test permit from the Office of Admissions on the same day.
    3. For applications filed thru regional filing centers or by mail, wait for your Test Permit to arrive in the mail. It will be mailed to your school. The list of examinees with processed applications will also be sent to your school’s official e-mail address. If your name is not among the listed examinees for your school, check with the Office of Admissions (9818500 Local 3827, 3828 or 3831), and follow-up with the UPCAT Examiner assigned to the UPCAT Test Center nearest to your high school on August 28 (the Friday just before the UPCAT).

 UPCAT RESULTS

  1. Check the UPCAT website (www.upcat.up.edu.ph) for announcements and posting of UPCAT Results (by February 2016). A list of qualifiers from your school will be sent to your Principal. Individual notices will also be sent to all examinees.
  2. If you qualify for admission to UP, register according to the instructions provided by the college or unit to which you have been accepted.
